Physical Day Information

2023 Sports Physical Day

Day: Saturday May 20th
Time: 8:00am until 10:00am
Cost: $30.00 Cash or Check
Location: Servite High School Gym

Athletes do not need to register for the event. Simply show up at the Servite gym on May 20 with $30 cash, and the medical history portion of the physical paperwork filled out. 

Should your athlete need an evaluation beyond the standard exam, Servite’s orthopedic surgeon and concussion specialist will also be on hand to address any concerns. This streamlined process ensures the sports medicine team fully understands the medical history of your athlete. The Servite Athletics Department can only accept physicals completed by an MD, DO, NP, or PA under the supervision of an MD or DO. Physical forms must also be stamped to be considered valid. If a physical does not meet these requirements it will be returned to the athlete. 

If you choose to complete physicals elsewhere, they must be completed and turned in prior to the first day of summer practice or your son will not be allowed to participate. Please verify that the physical is dated on or after May 15, 2023. This will ensure that your athlete is at least covered until the next Servite Physical Day is offered in May 2024.

 There are several options to submit your athlete’s physical:
1. Email a copy to the Head of Athletic Training at dboroian@servitehs.org
2. Drop off a hard copy to the Athletic Training Room
3. Drop off a hard copy to Ann Cross in the Athletics Office
DO NOT TURN IN YOUR ATHLETICS PHYSICAL INTO THE MAIN OFFICE. It may get filed there instead of being delivered to Athletics.
