Athletic Clearance
Every student planning on participating in athletics during the 2026-2027 school year must complete the Athletic Clearance process. The application includes contact information, emergency contact information, medical history, various waivers, and a place to upload your physical form. To begin the athletic clearance application, click the button below.
As you complete the clearance process, please be aware of the following:
- If you are a returning student, please log in to your existing account used in the previous school year.
- For school, please select Servite
- Student ID: If you are an incoming freshman or do not know your student ID, please type unknown in the field.
- Your physical must be completed by a MD or a DO.
- Hard copies of physicals will not be accepted by the athletic training staff or athletic department. All physicals must be uploaded through your clearance account.
Physicals
Physicals for the 2026-2027 school year need to be dated between May 26, 2026 and June 14, 2026.
Any current physicals dated outside of that range will need to be updated in order for the student-athlete to continue participating in any athletic activities.

PHYSICAL SITE PARTNERSHIP
Servite Athletics is proud to once again partner with Exer Urgent Care to provide additional opportunities for you to complete the required annual physical as part of the athletic clearance application. Exer Urgent Care offers two clinics in Anaheim and 10 total in Orange County. Exer is here to serve our student-athletes. The cost will be $40, and the proceeds will benefit Servite Sports Medicine. This offer is only valid through August 31, 2026.
