Athletic Clearance

Every student planning on participating in athletics during the 2024-2025 school year must complete the Athletic Clearance process. The application includes contact information, emergency contact information, medical history, various waivers, and a place to upload your physical form. To begin the athletic clearance application click the button below.

As you complete the clearance process, please be aware of the following:

If you are a returning student please log into your existing account used in the previous school year.
For school, please select Servite
Student ID: If you are an incoming freshman or do not know your student ID, please type unknown in the field.
Your physical must be completed by a MD or a DO.
Hard copies of physicals will not be accepted by the athletic training staff or athletic department. All physicals must be uploaded through your clearance account.

Physicals

Physicals for the 2024-2025 school year need to be dated between May 18, 2024 and June 17, 2024. Any current physicals dated outside of that range will need to be updated in order for the student-athlete to continue participating in any athletic activities.
