Transcripts
Effective May 1, 2008 the Records Office will have new office hours:
Monday thru Friday – 7:30AM - 4:00PM
In order to obtain official or unofficial transcripts, please contact Ms. Andrea Valenzuela in the Records Office at (714) 774-7575, extension 1117 or via email at avalenzuela@servitehs.org.
Requests can be faxed in to (714) 774-1404 attention: Ms. Valenzuela. Please provide the following information:
- FULL NAME (INCLUDING MIDDLE INITIAL)
- DATE OF BIRTH
- YEAR OF GRADUATION
- SIGNATURE (IMPORTANT)
- INDICATE WHERE TO SEND THE TRANSCRIPT (MAIL TO RESIDENCE, SCHOOL, EMPLOYER, ETC)
- INDICATE THAT PAYMENT WILL BE SENT VIA MAIL PAYABLE TO SERVITE HIGH SCHOOL.
Each official transcript is $4.00. Starting July 1, 2008 the cost will $5.00.
